Assessment checklists

Pre-Assessment Checklist

This checklist is a guide for both the assessor and the trainee and it specifically looks at

  • what unit standards are going to be assessed
  • the assessment conditions: (oral questions; written; open book; closed book or any other procedure)
  • the environment (particularly if it is in the work place and there are Health and Safety requirements where all the relevant staff have been informed)
  • the appeal process
  • whether the trainee is ready for the assessment

The assessor is responsible for checking that the assessment material meets the requirements of the unit standard(s) and should include

  • conditions that are clearly stated
  • instructions that are appropriate and easily understood
  • assessment enables learners to provide evidence corresponding with identified learning outcomes in performance criteria
  • assessment and marking schedules that indicate the range of evidence and judgments required to ensure consistency
  • questions that are aligned to the unit standard, elements and performance criteria

Post-Assessment Checklist

This checklist is a guide for both the assessor and the trainee to help confirm outcomes from the Pre-Assessment Checklist. This checklist will help determine whether the trainee has competency in the unit standard(s). If the trainee is assessed ‘Not Yet Competent’, the assessor and trainee will discuss and plan for further assessment to take place.

Observation Record

This document is part of the Assessment Guide, provided there is a practical component to the unit standard. This is to be used to observe and comment on a trainee in a practical situation for a particular unit standard requirement, in an element or performance criteria.

All checklists and the observation record must be completed by both the assessor and trainee. Please keep all checklists and the observation record with the other assessment materials as these will be required for moderation purposes.
